The Applicant(s) whose name appears above and signature appears below, hereby applies for membership in and agrees to purchase electric energy from SALEM ELECTRIC, PO Box 5588, Salem, Oregon, 97304 upon the following terms and conditions:
Application for membership will specify the premises on which electric energy obtained from Salem Electric would be used, and will be in such form as the Board of Directors of Salem Electric from time to time will prescribe. Such application will constitute an agreement by the applicant, if accepted for membership, to comply with and be bound by the Articles of Incorporation, Bylaws and rules and regulations of Salem Electric then in effect or as thereafter from time to time duly amended or promulgated, and also an agreement to obtain from or through Salem Electric the electric energy used by or for that member on the premises specified in the application.
Applicants for electric service are required to make certain advance arrangements for this service. Each Applicant for electric service may be required to sign Salem Electric’s form of application for electric service and supply Salem Electric with information relating to service requirements and the manner in which power will be utilized. Large industrial or commercial contracts may be written on a special form that will contain such provisions and stipulations as may be necessary or desirable to protect the interests of both Salem Electric and the customer. The “application for service” is a request for service only and does not, in itself, constitute a contract for service until Salem Electric actually delivers, or is ready to deliver, electric service to the customer. In the absence of a signed application for service, the delivery of service by Salem Electric, and its acceptance by the customer, will be deemed to constitute an agreement, and acceptance of Salem Electric’s rules and regulations. Applicants may apply for joint membership and will share equally in the rights and responsibilities of the membership. Upon acceptance of membership the applicant will pay a nonrefundable membership fee of one cent ($.01) which will entitle the applicant to membership.
- By signing this application, I give Salem Electric permission to run an identity verification check and soft credit inquiry.
- By providing Salem Electric your phone number, you are giving express written consent to call that phone number for Salem Electric business purposes.
- Salem Electric will use reasonable diligence to provide satisfactory and uninterrupted electric service, but cannot and will not guarantee such service, and will not be liable for injury, loss, or damage resulting from any failure or curtailment of electric service; nor will any failure or curtailment constitute a breach of contract. Whenever necessary, for the purpose of making repairs or improvements to its system, Salem Electric will have the right to temporarily suspend the delivery of electrical energy. In such cases, Salem Electric will give reasonable notice, if circumstances permit, and attempt to schedule its activities at the customer’s convenience. Salem Electric will not be liable for injury, loss or damage resulting from such suspension of the delivery of electrical energy.
- Meters will be installed on the outside of buildings or service structures, except, that in the case of rural or temporary service; a meter may be installed on a meter pole. Exceptions to this practice must be approved by Salem Electric. The customer will furnish a safe, convenient place, readily accessible, without risk of bodily harm to Salem Electric employees, and free from vibration, corrosive atmosphere and abnormal temperatures, in which to install the metering equipment.
- The member will take all reasonable and proper precautions to prevent damage or theft to Salem Electric’s property or facilities on the member’s premises. In the event that property of Salem Electric is damaged, Salem Electric may collect, from the responsible party or member responsible for account service, the cost of repairs or replacement.
