By signing this document I certify the information provided on this form is truthful and accurate. I have read and accept the program terms and conditions and understand that I may be subject to a verification audit by SMPA. I acknowledge that I must inform SMPA of my intention to install a renewable energy system and complete all documentation required by the utility. Rebates will not be issued until system is installed and permission to operate (PTO) has been granted by SMPA. Rebates may take up to 90 days to process.
