Radio-Controlled Switch Agreement
In order to assist in the conservation of electric power and to help slow the increasing cost of providing electric services, the undersigned Consolidated Cooperative member hereby agrees that a radio control device, including all related wiring and facilities, may be installed, operated and maintained on or near the member’s equipment located at the address shown below and such device may be connected to the member’s electric wiring.
Access to the switch for any maintenance or replacement of the switch is hereby granted by the member. Persons designated by Consolidated Cooperative are authorized to enter the member’s premises at reasonable times and upon proper identification in order to install, connect, maintain, relocate, repair, replace or remove such device. The device shall remain the property of Buckeye Power, Inc., and may be removed at the Cooperative’s discretion without liability.

The member understands that the device will permit the control of electric power requirements by temporarily switching off the unit during anticipated peak electric power usage periods.

Further, the member agrees to leave the switch installed and attached for a period of no less than five years in order to receive the rebate credit(s) as may be applicable at the time. Not permitting the installation or the removal of the switch could result in the rebate being withdrawn and payable in full to the Cooperative and/or a $50.00 removal fee.

The member represents that he or she is the owner of the premises identified above or that the owner has been identified at the place indicated above.
