I, as the undersigned member, request the following individual to be removed from my member record as a joint responsible party:

As the joint responsible party named above, I give my full consent to be removed from member record number noted above. I understand the removal of my name from this member record will waive any claim I may have against PVREA for any payment in the future for refund of any deposit, refund of any unclaimed funds, payment of capital credits past, present or future or any other amount that may become due to me from PVREA.

By typing my name here, I understand and agree that this form of electronic signature has the same legal force and effect as a manual signature. By signing this document you are stating the information you have provided is true and accurate and that you take full responsibility for any incorrect information. 
Member Staying On The Account Info
I understand, with the removal of the joint responsible party named above, I will now be solely responsible to PVREA for the payment of all amounts due including without limitation monthly utility bills and any delinquent amounts in accordance with PVREA's Articles of Incorporation, Bylaws, Tariffs, Rules and Regulations and terms of the Written Confirmation of Request for Electric Service as now in existence and as may hereafter be in existence from time to time.




The Member signing this document shall indemnify PVREA against, and hold PVREA harmless from any and all claims, actions, suits, proceedings, costs, expenses, damages and liabilities, including attorney's fees, arising out of, connected with, or resulting from PVREA acting to remove the above person as a responsible party, including without limitation the refund of any deposit, refund of any unclaimed funds, refund of capital credits or any other amount that may be due to the person whose name is removed.
